ABSTRACT

Introduction: forensic anthropology currently serves as a potent tool in primary and secondary identification, especially in mass disasters, increasingly common in our society. Objective: Verify the reproducibility of measured measurements. Proposes methods of personal identification. Method: to verify the reliability of the method, 25 radiographs were selected, the intra-class coefficient (ICC) and the Bland-Altiman statistical technique were calculated. accuracy of the method, was used. R studio and Medcalc were used. Results: The ICC was above 0.90 for all measurements. According to the Bland-Altman analysis, the average differences observed between examiners ranged from 0.017 (m5) to 0.020 (m7). These results highlight the reliability of measurements made by two different examiners performing the same tasks. Conclusion: The variables used in this study have shown satisfactory reproducibility. Thus, the selected and evaluated data can be used as metadata in automating the sex determination process from panoramic radiographs.


Introdução: A antropologia forense atualmente serve como uma ferramenta potente na identificação primária e secundária, especialmente em desastres em massa, cada vez mais comuns em nossa sociedade. Objetivo: Verificar a reprodutibilidade das medidas mensuradas. Propõe métodos de identificação pessoal. Método: para verificar a confiabilidade do método, foram selecionadas 25 radiografias, calculando-se o coeficiente intraclasse (ICC) e a técnica estatística de Bland-Altiman. Para avaliar a precisão do método, foram utilizados o R studio e o Medcalc. Resultados: O ICC foi superior a 0.90 para todas as medições. De acordo com a análise de Bland-Altman, as diferenças médias observadas entre os examinadores variaram de 0.017 (m5) a 0.020 (m7). Esses resultados destacam a confiabilidade das medições feitas por dois examinadores diferentes realizando as mesmas tarefas. Conclusão: As variáveis utilizadas neste estudo mostraram reprodutibilidade satisfatória. Assim, os dados selecionados e avaliados podem ser usados como metadados na automação do processo de determinação do sexo a partir de radiografias panorâmicas

ABSTRACT

Abstract Sex estimation is an important procedure in forensic anthropology for human identification. The development of new technologies, such as three-dimensional computed tomography (CT), has provided excellent alternatives for this purpose. This study examined and compared a morphological method for sex estimation using two different approaches - direct measurement of physical structures and tomographic analysis using 3D images. A total of 111 skulls from the Museum of Human Anatomy at the University of São Paulo (MAH-USP) were used, (60 males and 51 females). All specimens were scanned by Philips Brilliance 64 CT scanner equipment and their corresponding images were reconstructed in three-dimensional (3D) models. The morphological characteristics of the skulls were analyzed by an observer who was blinded to the sex of the specimens. Five cranial structures were analyzed: external occipital crest, mastoid process, supraorbital margin, glabella, and mental eminence. The structures were scored 1 to 5 according to Buikstra and Ubelaker and validated by Walker. The success rates of the sex estimates obtained through direct measurement of the dry skulls ranged from 67.4% to 70.4% as compared to 60.2% to 68.1% for CT reconstruction. When analyzed separately, the maximum accuracy of the method was 68.33% in males and 88.24% in females in the physical analysis of structures. The glabella and mastoid process were the most effective structures to estimate sex through both techniques, respectively. Our results show that 3D CT images can be accurately used in the morphological analysis for sex estimation, representing a viable alternative in forensic anthropology.

Abstract Food insecurity is a complex phenomenon that affects the health and wellbeing of vulnerable families. The aim of this study was to investigate the relationship between household food insecurity, dental caries, oral health-related quality of life, and social determinants of health among Indigenous adults. A cross-sectional study was conducted among Kaingang adults aged 35-44 years old from the Guarita Indigenous Land, Brazil. Food insecurity was assessed through the EBIA scale. Dental caries was assessed using the DMFT index. Participants answered the OHIP-14 questionnaire and a structured interview. Descriptive and multivariate analyzes using Poisson regression models were performed. The final sample included 107 adults from 97 households. Approximately 95% lived in food insecure families. Severe food insecurity was present in 58% of the households. The phenomenon was associated to the Bolsa Família benefit, household size, and greater perception of oral health impacts on quality of life. The high number of families affected by food insecurity reveals the social vulnerability of the Kaingang people. Food insecurity in Kaingangs adults is associated to oral health perception and social determinants of health.


Resumo Insegurança alimentar é um fenômeno complexo que afeta a saúde e o bem-estar de famílias vulneráveis. Este estudo objetivou investigar a relação entre insegurança alimentar domiciliar, cárie dentária, qualidade de vida relacionada à saúde bucal, e determinantes sociais de saúde entre indígenas adultos. Foi conduzido um estudo transversal com adultos Kaingang entre 35-44 anos da Terra Indígena Guarita, Brasil. Insegurança alimentar foi avaliada pela escala EBIA. Cárie dentária foi avaliada pelo índice CPOD. Participantes responderam o questionário OHIP-14 e uma entrevista estruturada. Foram realizadas análises descritivas e multivariadas usando modelos de regressão de Poisson. A amostra incluiu 107 adultos Kaingang de 97 domicílios. Aproximadamente 95% dos participantes viviam em famílias com insegurança alimentar. Insegurança alimentar grave esteve presente em 58% dos domicílios. O fenômeno foi associado ao Bolsa Família, densidade familiar e percepção dos impactos da saúde bucal na qualidade de vida. O alto número de famílias afetadas pela insegurança alimentar revela a vulnerabilidade social do povo Kaingang. Insegurança alimentar em adultos Kaingang está associada à percepção da saúde bucal e determinantes sociais da saúde.

ABSTRACT

Após o surgimento do Código de Defesa do Consumidor, a Odontologia passou por modificações jurídicas, fornecendo condições aos pacientes para cobrarem do profissional o cumprimento de suas obrigações. O objetivo do estudo foi analisar as decisões em primeira instância de processos julgados na área de Odontologia no ano de 2019 no Tribunal de Justiça de São Paulo (TJ-SP) em relação ao tempo de processo, especialidade do procedimento, tipo de pessoa requerida, tipo de justiça, classe do processo, realização de perícia, posicionamento do magistrado em relação ao laudo, tipo de obrigação estabelecida no processo, responsabilidade civil, pedido de reconvenção, e decisão judicial. Foi utilizado o acesso ao banco de sentenças do TJ-SP com os seguintes descritores no campo de busca: "dentista" e "responsabilidade profissional". Foram encontradas 234 sentenças. O tempo máximo entre o início do processo e o julgamento foi de 17 anos e o tempo mínimo, de 1 ano. A Implantodontia foi a especialidade mais processada (42,3%), e o dano moral foi a classe processual mais solicitada (29,9%). Em 75,6% das sentenças, a realização da perícia foi solicitada e, destes, em 70,9% dos casos o magistrado se embasou nas informações fornecidas no laudo para determinar sua sentença. A obrigação de resultado foi a mais associada à Odontologia pelos Magistrados, com estabelecimento da responsabilidade civil em 140 sentenças. Dessa forma, ressalta-se a importância do laudo pericial, visto que este atua de modo a esclarecer o magistrado, e consequentemente, se torna um fator essencial para guiar a decisão judicial.

ABSTRACT

Abstract Oral cancer is one of the most prevalent cancers in Brazil. An understanding of how public policies are implemented to address this problem can contribute to the construction of solutions. The "Programa de Melhoria do Acesso e da Qualidade" (PMAQ-AB and PMAQ-CEO) at the level of primary and secondary care are evaluations that also collect data on prevention and monitoring strategies for oral cancer. This study aimed to analyze the results of the incorporation of oral health teams to evaluate the strategies adopted by Brazil regarding the impact on the diagnosis of oral cancer. Of the 17,202 family health teams evaluated, 72.10% had oral health teams (OHT). Considering the strategies for prevention, screening, campaign, and follow-up of suspected cases, 72.27% of the campaign teams and 59.09% of the teams who accompanied suspected cases had OHT. In secondary care, more than 65% of the teams in the Southeast and South regions registered cases of oral cancer, and the referral network was more represented. The inclusion of OHT had a positive impact on campaign actions, follow-up, referral to specialists, and registration of suspected cases throughout Brazil.

Abstract The aim of this study was to verify the precision and applicability of two methods of age estimation, Kvaal's and Cameriere's methods, among Brazilian adults. A sample composed of periapical radiographs of canine teeth belonging to 320 Brazilian adults was analyzed, divided into groups according to sex (male and female) and age group (20-29 years, 30-39 years, 40-49 years and 50-59 years). Kvaal's method presented better results when compared to the results of Cameriere's method in the general evaluation of each canine tooth, except for the upper left canine (tooth 23), which presented a mean error (ME) with no statistically significant difference between the methods (Kvaal: ME = 7.43, p = 0.4991, Cameriere: ME = 7.55, p = 0.6982). In the evaluation by age groups, Kvaal's method presented a lower variation between the real age and estimated age when applied to the age groups of 20-29 years and 30-39 years compared to the results provided by Cameriere's method. With respect to the groups aged 40 to 49 years and 50 to 59 years, Cameriere's method presented better performance than the results provided by Kvaal's method. The methods of estimating age proposed by Kvaal and Cameriere are simple and nondestructive and have demonstrated reproducibility and reliability. The Kvaal method was more accurate for the age groups of 20-29 and 30-39 years, and for those over 40 years, the Cameriere method was the most accurate.

ABSTRACT: Introduction: The epidemiological profile of dental caries for Indigenous Peoples is complex and heterogeneous. The oral health of the Kaingang people, third largest Indigenous population from Brazil, has not been investigated so far. Objective: The purpose of this study was to assess the prevalence and severity of dental caries, in addition to the associated factors of the need of dental extraction among Kaingang adult Indigenous. Methods: A cross-sectional oral health survey was conducted among Kaingang adults aged from 35 to 44 years old living in the Guarita Indigenous Land, Rio Grande do Sul. Clinical exams were performed to analyze the conditions of dental crown and treatment needs. Results: A total of 107 Indigenous adults were examined. Mean DMFT score was 14.45 (± 5.80). Two-thirds of the DMFT score accounted for missing teeth. Anterior lower dentition presented the highest rates of sound teeth, whereas the lower first molars had the lowest. Need for dental extraction was observed in 34.58% and was associated with village location, time of last dental visit, and higher number of decayed teeth. Conclusion: The high frequencies of caries and missing teeth observed in this population indicate a lack of adequate assistance. It is essential to discuss health care models in order to combat avoidable social and health injustices.


RESUMO: Introdução: O perfil epidemiológico de cárie dentária dos povos indígenas é complexo e heterogêneo. A saúde bucal do povo Kaingang, terceira maior população indígena do Brasil, ainda não foi investigada. Objetivo: O objetivo deste estudo foi avaliar a prevalência e severidade de cárie, além dos fatores associados à necessidade de extração dentária entre adultos Indígenas Kaingang. Métodos: Foi realizado um inquérito de saúde bucal entre adultos Kaingang com idade entre 35 e 44 anos residentes na Terra Indígena Guarita, Rio Grande do Sul. Exames clínicos foram realizados a fim de analisar as condições da coroas dentárias e as necessidades de tratamento, seguindo os critérios e diretrizes da Organização Mundial da Saúde e da Pesquisa Nacional de Saúde Bucal SB Brasil 2010. Resultados: O total de 107 Indígenas Kaingang foi examinado. O índice de dentes cariados, perdidos e obturados (CPOD) médio observado foi de 14,45 (± 5,80). Dois terços do escore do indíce foram compostos do componente "perdidos". A dentição inferior anterior apresentou as maiores taxas de dentes hígidos, enquanto os primeiros molares inferiores apresentaram as menores. Necessidade de extração dentária foi observada em 34,58%, sendo associada com a localização da aldeia, tempo da última consulta odontológica e maior número de dentes cariados. Conclusão: As altas frequências de cárie não tratada e dentes perdidos observados nessa população indicam a falta de assistência adequada. É necessário discutir modelos de atenção à saúde para combater iniquidades sociais e de saúde.

Aim: to analyze the prevalence of different definitions of functional dentition, prosthodontic status and associated factors, in an indigenous population from Brazil. Methods: a cross-sectional oral health survey was conducted with Indigenous adults aged 35-44 years. A single examiner collected clinical data through oral examinations and sociodemographic data using a structured questionnaire. Dentitions were classified according to four classification systems of functional dentition: FDWHO (> 20 teeth), FDGROUP2 (> 10 teeth in each arch), FDGROUP3 (all anterior teeth), and FDGROUP4 (> 10 teeth in each arch, all anterior teeth, and sufficient posterior region). Use and need of prosthodontics was also evaluated. Uni and multivariate analysis were conducted at the level of significance of 5%. Results: Indigenous adults presented considerably low frequencies of prosthodontic use and functional dentition, independently of the definition analyzed. Substantial differences of prevalence rates were observed among the four definitions of functional dentition, ranging from 48.62% to 11.93%. Age and municipality were associated with use of dental prosthesis and prosthodontic need, respectively. Significant discrepancies in functional dentition rates were observed regarding sex and time of the last dental appointment. Conclusions: Indigenous adults are severely affected by tooth loss and, consequently, by low frequencies of functional dentition. The scenario is worsened by the elevated need of the population for prosthodontics. The phenomenon was associated with age, sex, access to specialized dental care and time of the last dental visit

Abstract In the identification process, the foramen magnum has shown controversial results in sex estimation. The present study aimed to analyze the morphometric variables of the foramen magnum in Brazilian adult cranium for sex estimation. The sample was composed of 100 craniums (53 males and 47 females) from the documented collection of the Institute of Teaching and Research in Forensic Sciences. The protocol measurement was constituted of two linear measurements: maximum length of the foramen magnum and maximum breadth of the foramen magnum and two formulas to calculate the area, method one (M1) and method two (M2). Descriptive statistics showed statistically significant differences between sex (p<0.05) for all variables. The univariate discriminant functions showed an accuracy between 56.0-62.0%, and the multivariate analysis showed a percentage of accuracy between 60.0-65.0%, the greatest accuracy was found combining the two linear measurements with M1(71.7%), even after cross-validation (66%). Receiver Operating Characteristic (ROC) curve analysis showed that M2 is the best parameter for estimating sex (AUC=0.693). A reference table for Brazilians using the foramen magnum parameters was developed based on the results of the ROC curve analysis. In conclusion, the foramen magnum should be used with caution to estimate sex in forensic cases of fragmentary craniums, due to the limited accuracy.


Resumo No processo de identificação humana, a análise do forame magno apresenta resultados controversos para estimativa do sexo. O presente estudo teve como objetivo analisar as variáveis morfométricas do forame magno em crânios adultos brasileiros para estimação sexual. A amostra foi composta por 100 crânios (53 sexos masculino e 47 sexo feminino) pertencentes a coleção osteológica documentada do Instituto de Ensino e Pesquisa em Ciências Forenses. A medida do protocolo foi constituída por duas medidas lineares: comprimento máximo do forame magno e largura máxima do forame magno e duas fórmulas para calcular a área, método um (M1) e método dois (M2). As estatísticas descritivas evidenciaram diferenças estatisticamente significativas (p<0,05) para todas as variáveis. As funções discriminantes univariadas apresentaram uma porcentagem de acerto entre 56,0-62,0% e a análise multivariada mostrou uma porcentagem de acerto entre 60,0-65,0%. A análise da curva ROC apontou que M2 é o melhor parâmetro para estimar o sexo (AUC=0,693). Uma tabela de referência para brasileiros que utilizam os parâmetros do forame magnum foi desenvolvida com base nos resultados da análise da curva ROC. Em conclusão, o foram e magnum deve ser usado com precaução para estimar o sexo em casos forenses de cranio fragmentado, devido à precisão limitada.

Aim: The present investigation intended to compare the craniometric variations of two samples of different nationalities (Brazilian and Scottish). Materials and methods: The Brazilian sample consisted of 100 modern complete skulls, including 53 female skulls and 47 male skulls, and the Scottish sample consisted of 100 historical skulls (61 males, 39 females) and 36 mandibles (24 males, 12 females). The cranial measurement protocol was composed of 40 measurements, 11 bilateral and 29 unilateral, and the measurement protocol of the mandible was composed of 15 measurements, with six that were bilateral and nine that were unique. The comparative analysis of the metric variability between the two samples was performed using the means and medians analysis, the t-test, the Wilcoxon test, and the coefficient of variance, with a significance level of 5%. Results: The results showed that, among the 72 analysed variables, 44 measurements (61.11%) presented statistical differences between the samples. The Scottish skull tends to have a cranial length (GOL diff=5.53), breadth (XCB diff=3.78) and height (NPH diff=5.33) greater than the Brazilian skulls, and the Scottish mandibles tend to show a higher mandibular ramus height (MRH diff=9.25), a higher mandibular body height (HMB diff=6.37) and a larger bigonial breadth (BGB diff=5.29) than the Brazilians. The discriminant analysis of the 51 cranial measurements and 21 mandibular measurements showed a variation of the percentage of accuracy between 46.3-83.8%. Conclusion: The metric analysis demonstrated that there is variability between the two samples studied (61,11%), but a concrete cause cannot be determined considering the multifactorial aspects of the variations of form and size

Abstract Oral mucositis is a painful condition that occurs in patients who have undergone haematopoietic stem-cell transplantation (HSCT) and has a huge impact on their quality of life. The objective of this study was to examine the extent to which interleukin-6 (IL-6) and tumour necrosis factor alpha (TNF-α) are associated with oral health quality of life among patients who underwent HSCT. A. C. Camargo Cancer Center patients were evaluated over 20 months at 4 different time-points: a) at patient admission (M1); b) on the day of infusion of HSC (M2); c) 12 and 20 days after the first day of the conditioning regimen for autologous and allogeneic transplantation, respectively (M3); and d) 30 days after the first day of the conditioning regimen (M4). Mucositis clinical evaluations were performed using World Health Organization (WHO) criteria. Oral health quality of life was measured using the Oral Health Impact Profile short form (OHIP-14), Oral Mucositis Quality of Life (OMQoL) and Patient-Reported Oral Mucositis Symptom (PROMS) scales. Correlations between clinical data and quality of life scores were examined. STATA 11.0 was used to perform the statistical analyses (5% level of significance). Eighty-two patients participated in the study; 62.2% were male, 28.05% had multiple myeloma, and the mean age was 48.49 years (SD 13.76). Higher scores (worse quality of life) were observed as mucositis scores increased, but the results were not significant. The OMQoL had a high correlation with OHIP-14 (0.8377), but the correlation between the PROMS scale and the OHIP-14 was lower (0.6643). Higher concentrations of IL-6 and TNF-α were associated with worse quality of life, according to all indices (p < 0.05). Oral mucositis was associated with quality of life scores (p < 0.01).

Abstract Questionnaires for the assessment of knowledge and self-perception can be useful to diagnose what a dentist knows about management and administration. The aim of the present study was to create and validate the Dental Management Survey Brazil (DMS-BR) scale, based on meetings with experts in the field. After having elaborated the first version, 10 audits were performed in dental offices in order to produce the final version, which included nine dimensions: location, patient, finance, marketing, competition, quality, staff, career, and productivity. The accuracy of the instrument was measured by intrarater and interrater reliability. In the validation phase, 247 Brazilian dentists answered a web-based questionnaire. The data were processed using Stata 13.0 and the significance level was set at 95%. The instrument had intrarater and interrater reliability (ICC-0.93 and 0.94). The overall average of respondents for the DMS-BR scale was 3.77 (SD = 0.45). Skewness and kurtosis were below absolute values 3 and 7, respectively. Internal validity measured by Cronbach’s alpha was 0.925 and the correlation of each dimension with the final result of the DMS-BR ranged between 0.606 and 0.810. Correlation with the job satisfaction scale was 0.661. The SEM data ranged between 0.80 and 0.56. The questionnaire presented satisfactory indicators of dentists’ self-perception about management and administration activities.

Abstract The objective of this investigation was to compare the accuracy of mandibular measurements using a stabilizer (MS) with gold standard computed tomography (GS) images. Sixty mandibles were studied. Werth TomoScope HV Compact® was used to obtain CT images (GS), and the MS was also used. Analysis of the CT scans was performed using the VG Studio Max software® (Volume Graphics GmbH, Heidelberg, Germany), and MS was used after the proper positioning of the mandible. Descriptive and paired t test measures were used, and a ROC curve was calculated, as well as sensibility and specificity. MedCalc and STATA 13.0® were used (95% level of significance). Bicondylar breadth, bicoronoid breadth and minimum ramus breadth reached the highest concordance correlation coefficients at 0.99 (0.99-1.00), 0.99 (0.99–1.00) and 1.00 (0.99–1.00), respectively. Comparing observers with GS, the lowest accuracy was noted for the maximum mandibular length [0.59 (0.45–0.69), 0.64 (0.51–0.74)], the breadth of the right (0.14 (0.04–0.23), 0.14 (0.004–0.24)) and left mandibular body [0.14 (0.03–0.24), 0.16 (0.05-0.26)], and the right [0.58 (0.45–0.69), 0.63 (0.51–0.73) and left (0.59 (0.45–0.70), 0.59 (0.46–0.69)] mandibular angle. Various measurements exhibited good sensibility for males using MS: maximum mandibular length (78.12), bicondylar breadth (78.12), left mandibular notch breadth (84.37), and the left height of the mandibular body at the mental foramen (75.00). High specificity in discriminating females was observed for the left maximal ramus height (85.19), mandibular length (85.71), bicoronoid breadth (96.43), right height of the mandibular body at the mental foramen (82.19), bimental breadth (78.57), breadth right (92.86) and left (96.43) mandibular body, minimum ramus breadth (89.29), and left mandibular angle (85.71). MS was able to discriminate sexual dimorphism.

ABSTRACT

Forensic Odontology is a branch of Dentistry that works with the Law in assisting Justice and the professional involved in this specialty helping the identification of human remains, in various situations ­ charred corpses, cadavers and unidentified bodies. The aim of this paper was to list and analyze the number and types of teeth present in cadavers submitted to the Forensic Anthropology Lab of the Forensic Medicine Center (Ribeirão Preto), from 2005 to 2010, in order to evaluate and compare the importance of each dental element in human identification. During the research, the Lab received, inserted or submitted with the cadavers, for anthropological analysis, 588 teeth, of which 303 were upper teeth and 285 lower teeth. The upper teeth mostly found were the first and second molars (n=48) and the mostly found lower teeth were canines and first bicuspids (n=44). The most absent upper tooth ante mortem was the third molar (n=25) and, on the lower arch, the first molar (n=33). In contrast, the most absent teeth post mortem on the upper dental arch were central and lateral incisors (n=23 and n=20, respectively) and on the lower arch the same dental elements (central incisors - n=37, lateral incisors ­ n=28). The techniques for estimating age, ancestry, stature and gender by analysis of dental characteristics varied according to the population. In Brazil, due to the wide population miscegenation, the techniques mentioned may not present the same level of reliability, making it necessary to associate the dental exam to the anthropological analysis, in order to reach the most scientifically reliable results. It was concluded that, due to the importance of the dental elements in the process of human identification, it is necessary the presence of a dental surgeon in the team to collect the remains and perform the identification.


A Odontologia Forense é um ramo da odontologia que trabalha com a Lei na assistência à Justiça e ao profissional envolvido nessa especialidade especialmente ajudando na identificação de restos humanos, em várias situações - corpos carbonizados, cadáveres e corpos não identificados. O objetivo deste trabalho foi o de listar e analisar o número e tipos de dentes presentes em cadáveres submetidos a exame no Laboratório de Antropologia Forense do Centro de Medicina Legal (Ribeirão Preto), de 2005 a 2010, a fim de avaliar e comparar a importância de cada elemento dental para identificação humana. Durante a pesquisa, o laboratório recebeu 588 dentes inseridos ou juntos aos cadáveres para análise antropológica, destes 303 eram dentes superiores e 285 dentes inferiores. Os dentes superiores mais encontrados foram os primeiros e segundos molares (n = 48) e os dentes inferiores mais encontrados foram os caninos e pré-molares (n = 44). O dente superior com maior ausência ante-mortem foi o terceiro molar (n = 25) e, na arcada inferior, o primeiro molar (n = 33). Em contrapartida, o dente mais ausente post-mortem na arcada dentária superior foram os incisivos centrais e laterais (n = 23 e n = 20, respectivamente) e na arcada inferior os mesmos elementos dentários (incisivos centrais - n = 37, incisivos laterais - n = 28). As técnicas para estimar a idade, ascendência, estatura e sexo através da análise das características dentárias variou de acordo com a população. No Brasil, devido à ampla miscigenação da população, as técnicas mencionadas podem não apresentar o mesmo nível de confiabilidade, tornando-se necessário associar o exame odontológico à análise antropológica, a fim de alcançar resultados mais confiáveis cientificamente. Concluiu-se que, devido à importância dos elementos dentários no processo de identificação humana, é necessária a presença de um cirurgião-dentista na equipe para recolher os restos humanos e executar a identificação.

ABSTRACT

The objectives of this study were to develop a specific formula to estimate age in a Brazilian adult population and to compare the original formula from Cameriere to this Brazilian formula. The sample comprised 1,772 periapical radiographs from 443 subjects (219 men, 224 women) that were organized into 12 groups according to sex (men or women) and age (20-29, 30-39, 40-49, 50-59, 60-69, and 70 years and older). The films were analyzed using the criteria described by Cameriere et al. (2004) and Adobe Photoshop®. We obtained a mean error of 8.56 (SD = 5.80) years for tooth 13, 7.99 (SD = 5.78) years for tooth 23, 8.38 (SD = 6.26) years for tooth 33, and 8.20 (SD = 6.54) years for tooth 43. When teeth were combined in the analysis, we observed lower mean errors. The Brazilian formula developed from this sample group was more accurate than Cameriere’s formula. However, other factors must be considered to improve age estimates in adults.

OBJECTIVE: To analyse recent trends in oral cancer mortality, focusing specifically on differences concerning gender and race. METHODS: Official information on deaths and population in the city of Sao Paulo, 2003 to 2009, were used to estimate mortality rates from oral cancer (C00 to C10, International Classification of Diseases, 10th Revision), adjusted for age and stratified by gender (females and males) and race (blacks and whites). The Prais-Winsten auto-regression procedure was used to analyse the time series. RESULTS: During the study period, 8,505 individuals living in the city of Sao Paulo died of oral cancer. Rates increased for females (rate of yearly increase = 4.4%, 95%CI 1.4;7.5), and levelled off for men, which represents an inversion of previous trends among genders in the city. Increases were identified for blacks, with a high rate of yearly increase of 9.1% (95%CI 5.5;12.9), and levelled off for whites. Oral cancer mortality in blacks almost doubled during the study period, and surpassed mortality in whites for almost all categories. CONCLUSIONS: Mortality presented a higher increase among women than in men, and it doubled among backs. The surveillance of trends of oral cancer mortality across gender and racial groups may contribute to implementing socially appropriate health policies, which concurrently reduce the burden of disease and the attenuation of unfair, avoidable and unnecessary inequalities in health. .


OBJETIVO: Analisar tendências recentes da mortalidade por câncer de boca, focalizando diferenças quanto a sexo e raça. MÉTODOS: Foram utilizados dados oficiais de população e mortalidade na cidade de São Paulo entre 2003 e 2009 para estimar coeficientes de mortalidade por câncer de boca (C00-C10, Classificação Internacional de Doenças, 10a Revisão), com ajuste por idade e estratificação por sexo (feminino e masculino) e cor (negros e brancos). Para cálculo de tendências foi utilizado o procedimento de Prais-Winsten de autorregressão para análise de séries temporais. RESULTADOS: No período de estudo, 8.505 indivíduos residentes na cidade morreram de câncer de boca. Os coeficientes apresentaram tendência crescente para mulheres (taxa de crescimento anual = 4,4%, IC95% 1,4;7,5) e estacionária para homens, representando inversão das tendências anteriores entre os sexos, na cidade. Identificou-se tendência crescente para negros, com elevada taxa de aumento anual (9,1%, IC95% 5,5;12,9), e tendência estacionária para brancos. A mortalidade por câncer de boca em negros quase dobrou durante o período e ultrapassou a mortalidade em brancos para quase todas as categorias. CONCLUSÕES: A mortalidade entre mulheres aumentou mais do que entre homens e duplicou entre negros. A vigilância de tendências da mortalidade por câncer de boca entre os estratos de sexo e cor pode contribuir para programas de saúde que reduzam a carga de doença e atenuem diferenças em saúde que são injustas, evitáveis e desnecessárias. .

La mortalidad por cáncer de boca en negros casi se duplicó durante el período y sobrepasó la mortalidad en blancos para casi todas las categorías. CONCLUSIONES: la mortalidad entre mujeres aumentó más que en hombres y se duplicó en negros. La vigilancia de tendencias de mortalidad por cáncer de boca entre los estratos de sexo y raza puede contribuir en programas de salud que reduzcan la carga de enfermedad, y atenúen las diferencias que son injustas, evitables e innecesarias. .

ABSTRACT

A epidemiologia estuda quantitativamente a distribuição dos fenômenos de saúde/doença, e seus fatores relacionados, em populações definidas. O objetivo desse trabalho foi elaborar três textos sistematizados abordando a epidemiologia em três campos da Odontologia. O conceito de texto sistematizado foi introduzido nos concursos de Livre-Docência no artigo 82 do Estatuto da USP e o presente trabalho está organizado de acordo com o modelo estabelecido e seguiu a seguinte ordem: introdução, histórico, textos da sistematização, conclusão, textos selecionados e referências. Foram apresentados três textos sistematizados: 1) Método epidemiológico e pesquisas aplicadas em saúde bucal, qualidade de vida e fatores associados; 2) Métodos de pesquisas epidemiológicas em câncer, especialmente câncer de boca; 3) Uso da epidemiologia em ciências forenses. Para a elaboração do texto sistematizado 1, foram utilizados 7 artigos que tiveram a participação do autor e que estão publicados. Para o segundo texto, foram apresentados 8 trabalhos científicos. O terceiro texto contém 5 artigos científicos e 2 pedidos de patente. Pudemos concluir que a epidemiologia tem tem sido amplamente utilizada em Odontologia. Novas técnicas estatísticas e abordagem multivariáveis e multiníveis têm ajudado a explicar as condições de saúde bucal. Cada área específica, tais como o estudo da qualidade de vida, o câncer e boca e as ciências forenses tem desenvolvido técnicas específicas, existindo ainda campo para novas aplicações.


The epidemiological field studies quantitatively the distribution of health/illness phenomena, and its related factors in specific populations. The objective of this investigation was to elaborate three systematic texts highlighting the epidemiology field in three Odontology areas. The systematic text conceptwas introduced into the associate professor carreer through the USP Estatuto article 82 and this document is organized according to the established model, as following: introduction, background, systematic texts, conclusion, selected papers and references. Three systematic texts are presented: 1) Epidemiological method and investigations applied to oral health, quality of life and associated factors; 2) Epidemiological investigations methods on cancer, especially oral cancer; 3) Epidemiological methods on forensic sciences. In the systematic text 1, 7 published papers that had the author's participation were included. In the second text, 8 scientific papers were presented. The third text has 5 papers and 2 patents. It was concluded that the epidemiology has been widely used in Dentistry. New statistic techniques and multivariable and also multilevel analyses haveaided to explain oral health conditions. Each specifc área, as per quality of life, oral cancer and forensic sciences has developed specific techniques, and there's severa I possibilities to new applications.

ABSTRACT

Apesar dos inúmeros avanços no conhecimento científico do câncer de boca até o momento, muito ainda há para ser corretamente descoberto e compreendido. Convicções do passado foram substituídas por novas evidências científicas. O câncer de boca acomete cerca de 275.000 pessoas no mundo anualmente sendo mais de 14.100 casos no Brasil, em 90% dos casos é diagnosticado como carcinoma epidermoide. A taxa de mortalidade permanece praticamente a mesma de décadas passadas. Suas características clínicas são bastante conhecidas, mas apesar disso o número de pacientes diagnosticados tardiamente permanece alto. A biologia celular avançou muito, porém ainda carece de um marcador eficiente. A biópsia permanece como o padrão ouro do diagnóstico, apesar das pesquisas realizadas com outras técnicas. O tratamento odontológico de pacientes portadores de neoplasias malignas será cada vez mais comum nos consultórios e o Cirurgião-Dentista deverá estar preparado para diagnosticar e lidar com essas situações


Although the advances in the oral cancer knowledge, several issues have to be addressed for a better understood of this disease. Convictions of the past have been replaced by new scientific evidences. Oral cancer affects annually more than 275.000 people worldwide and more than 14.100 cases in Brazil, 90% of these are squamous cell carcinoma; the mortality rate remains the same for many decades. Their clinical characteristics are well known, but despite this, the number of patients with late diagnosis remains high. Cell biology had a great advance in the last years and a precise marker is still a dream. Biopsy remains the gold standard for diagnosis of oral cancer in spite of studies with other techniques. The dental treatment of oncologic patients will be more common and the dentist should be prepared to diagnose and manage these patients

OBJETIVO: Avaliar a validade discriminante do questionário de qualidade de vida da Universidade de Washington para pacientes com câncer de cabeça e pescoço e identificar possíveis fatores sociodemográficos que modifiquem seus resultados. MÉTODOS: Foram entrevistados 47 pacientes com câncer de boca e orofaringe em estágio pré-cirúrgico em um hospital no sul do município de São Paulo em 2007, e 141 pacientes sem câncer, pareados por sexo e idade em uma proporção de três para um, que foram atendidos em ambulatórios do mesmo hospital em 2008. Os resultados obtidos para os dois grupos foram comparados pelo teste t de Student. Para os pacientes sem câncer utilizou-se análise de regressão de Poisson para avaliar possíveis fatores de modificação da qualidade de vida. RESULTADOS: O escore geral de qualidade de vida foi significantemente mais elevado (p < 0,001) para os pacientes sem câncer (91,1) do que para os pacientes com câncer (80,6). Observações análogas foram efetuadas para oito dos doze domínios de qualidade de vida compreendidos no questionário (dor, aparência, deglutição, mastigação, fala, ombros, paladar e ansiedade). Como possíveis fatores de modificação dos escores de qualidade de vida foram identificados renda familiar (com impacto nos domínios de recreação, p = 0,017, e função dos ombros, p = 0,049), escolaridade (em ansiedade, p = 0,003), sexo (em função dos ombros, p = 0,038) e dor de dente (em mastigação, p = 0,015). CONCLUSÕES: O questionário tem validade discriminante, pois seus escores são especificamente mais afetados para pacientes com câncer. Reforça-se a indicação do questionário para monitorar o tratamento e recomenda-se avaliar os fatores que podem causar impacto nesses indicadores.


OBJECTIVE: To assess discriminant validity of the University of Washington quality of life questionnaire for patients with head and neck cancer, and to identify socio-demographic factors that may modify its results. METHODS: We interviewed 47 patients with oral and oropharyngeal cancer in pre-surgical stage in a hospital located in the South region of the city of São Paulo, in 2007, and 141 patients without cancer, matched by sex and age in a ratio of three to one, who were attending outpatient clinics of the same hospital in 2008. The results for the two groups were compared by the Student t test. Poisson regression models to evaluate factors that may modify quality of life scores among patients without cancer. RESULTS: The overall quality of life score was significantly higher (p < 0.001) for patients without cancer (91.1) than for patients with cancer (80.6). Similar observations were made for eight of the twelve quality of life domains included in the questionnaire (pain, appearance, swallowing, chewing, speech, shoulder, taste, and anxiety). As factors that may modify the quality of life scores, we identified family income (which impacted in recreation, p = 0.017, and shoulder function, p = 0.049), schooling (in anxiety, p = 0.003), sex (in shoulder function, p = 0.038) and toothache (in chewing, p = 0.015). CONCLUSIONS: The questionnaire has discriminant validity, because its scores are specifically more reduced among cancer patients. The use of the questionnaire for monitoring the treatment of cancer patients is reinforced, and the assessment of factors that may impact in its results is recommended.

ABSTRACT

Objetivo. Avaliar a relação entre alimentos de origem animal e câncer de boca e orofaringe.Métodos. Estudo caso-controle, de base hospitalar, pareado por sexo e idade (± 5 anos) coma coleta de dados realizada entre julho de 2006 e junho de 2008. A amostra foi composta por296 pacientes com câncer de boca e orofaringe e 296 pacientes sem histórico de câncer atendidosem quatro hospitais da cidade de São Paulo (SP), Brasil. Foi aplicado um questionário semiestruturado,para a coleta de dados relativos à condição socioeconômica e aos hábitos deletérios(tabaco e bebidas alcoólicas). Para avaliação do consumo alimentar, utilizou-se um questionáriode frequência alimentar qualitativo. A análise se deu por meio de modelos de regressão logísticamultivariada, que consideraram a hierarquia existente entre as características estudadas.Resultados. Entre os alimentos de origem animal, o consumo frequente de carne bovina(OR = 2,73; IC95% = 1,27–5,87; P < 0,001), bacon (OR = 2,48; IC95% = 1,30–4,74;P < 0,001) e ovos (OR = 3,04; IC95% = 1,51–6,15; P < 0,001) estava relacionado ao aumentono risco de câncer de boca e orofaringe, tanto na análise univariada quanto na multivariada.Entre os laticínios, o leite apresentou efeito protetor contra a doença (OR = 0,41; IC95% =0,21–0,82; P < 0,001).Conclusões. O presente estudo sustenta a hipótese de que alimentos de origem animalpodem estar relacionados à etiologia do câncer de boca e orofaringe. Essa informação podeorientar políticas preventivas contra a doença, gerando benefícios para a saúde pública.


Objective. Evaluate the relationship between animal-derived foods and mouth andoropharyngeal cancer.Methods. Hospital-based case-control study matched by sex and age (± 5 years) withdata collected between July of 2006 and June of 2008. The sample contained 296 patientswith mouth and oropharyngeal cancer and 296 patients without a cancer history who weretreated in four hospitals in the City of São Paulo, State of São Paulo, Brazil. A semistructuredquestionnaire was administered to collect data regarding socioeconomic condition andharmful habits (tobacco and alcoholic beverage consumption). To assess eating habits,a qualitative questionnaire that asked about the frequency of food consumption wasused. The analysis was rendered by means of multivariate logistic regression models thatconsidered the existing hierarchy among the characteristics studied.Results. Among foods of animal origin, frequent consumption of beef (OR = 2.73;CI95% = 1.27–5.87; P < 0.001), bacon (OR = 2.48; CI95% = 1.30–4.74; P < 0.001) andeggs (OR = 3.04; CI95% = 1.51–6.15; P < 0.001) was linked to an increased risk ofmouth and oropharyngeal cancer, in both the univariate and multivariate analyses.Among dairy products, milk showed a protective effect against the disease (OR =0.41; CI95% = 0.21–0.82; P < 0.001).Conclusions. This study affirms the hypothesis that animal-derived foods can beetiologically linked to mouth and oropharyngeal cancer. This information can guidepolicies to prevent these diseases, generating public health benefits.
